Admissions Path

1. Gather School Record Requirements

  • Pre-K-12 Grade Students: Submit current and recent report cards, school records (last two years of report cards and standardized test scores). If applicable, include a transcript and any educational or diagnostic testing.
  • Disciplinary Records: Disciplinary records should be provided if applicable. A behavioral referral will be requested from the current school for all Pre-K-12 grade students.

2. Gather Personal Record Requirements

  • Birth Certificate: A copy is required for all applicants
  • Immunization Record: Submit the most current immunization record, including the full date of each shot and the provider’s signature or stamp
  • Parent/Guardian Information Form: For step-parents or families with a second household
  • Special Testing Reports: Provide copies of any physical or psychiatric testing reports
  • Pastoral Recommendation: Must be completed by your home church
  • Teacher Recommendation Form (if applicable)
  • Custodial Arrangements: Families with specific custodial arrangements must submit the most recent court documents regarding custody and educational decisions, signed by a judge and notarized

3. Entrance Screening

  • PK3-4th Grade: Students will be required to complete a school readiness assessment
  • 5th-6th Grade: Students will be required to take the IXL Diagnostic Assessment to evaluate their readiness for grade-level standards-based skills
  • 7th-12th Grade: Students who do not have current standardized testing results will be required to take a MAP assessment for math and reading
  • Homeschool Applicants: All homeschool applicants must complete a placement test as well

4. Fill Out An Application

  • This form will require all requested records and documents. Documentation may be shipped to 1003 N 129th E Ave, Tulsa, OK 74116. Forms may also be faxed to (918) 514-7798 or emailed to info@lincolnchristianschool.com

  • We are accepting applications for the 2025-26 & 2026-27 school year.

    The 2025-26 form requires a non-refundable $24.00 application fee per student.

    The 2026-27 form requires a non-refundable $75.00 application fee per student.
